Mom cory paleatus belly up

Bummer of a day :(
My Mommy cory had been having issues for a while now but I thought someone might be able to shed light on what happened.
About 3 weeks ago, she developed a tiny 'skinned' spot on her head. I thought it might be from brushing it against a piece of slate or something. Treated with melafix. It seemed to get better for a while and them after the treatment, it came back but them looked like a pointy hole. Like if you bonked your head on the corner of a cube. Didn't look at all like pictures I saw of 'hole-in-the-head disease'.
Her one eye became puffy last night and I started melafix again. Today she was in fish-heaven and the "spot" was perhaps noticed by other hungry fish :shock: because it had become a bit of a mess if you know what I mean.

Any idea was might cause that funny dent/hole sort of thing? I checked all the slate edges, nothing even vaguely sharp or pointy.

On the bright side, I still have many of her kids :)

Watching all the others carefully and there's a slight indentation on the others in the same spot, just behind the eyes and up a bit. Nothing look wrong there but that's exactly where the other trouble started.
Anyone know anatomically what that little indents are? It where ears would be I suppose :?:
