My Cory catfish has lost his face

My cory catfish is losing his face, it sounds crazy I know.
I have fluorite substrate and I think this is the cause of the injury to my fish. I don't know how this fish is alive. I check the water weekly and the ammonia and nitrite are at 0. The nitrate can get a little elevated around 40 maybe, but I've done a lot of water changes. The reason why I brought it up is because there's no warning on those bags, that I remember seeing, for a fish like cory catfish and I'm getting ready to break down another tank and combine the two, which would put more cory catfish in this tank. I will be changing out the substrate before this happens.
The guys at the fish store don't agree with me that it could be the substrate they say it's some kind of bacterial infection, I disagree. The other fish in this tank consist of 4 giant tetras and 3 head and tail lights, that's it. I'm trying to solve the cause of this injury to my fish before adding more fish. I don't know how this guy eats, if at all at this point. He has been like this for awhile with it just getting worse. He doesn't forage anymore but still swims and goes to the top for air.
I think that substrate is the cause and they need warnings on the bags so this doesn't happen again. What a task and waste of money and such a sad lesson.
I will be changing this back to regular small gravel as my plants did fine in that anyway. I thought it would be good for the plants. Bad idea.

Cory's dont do well on sharp edges. Once an injury sets in it can be hard to cure. Example a sharp edge puts a small cut in a barbel bacteria infection follows.

One of the more experianced at recovering from this sort of injuries might advise more but the fish may not be able to feed at present.

Yes I wouldn't have thought that this would be an injury that could be fixed.
Fortunately and unfortunately this fish did pass away yesterday.
I am changing out the fluorite for fine gravel before this happens to another fish.
