That's certainly not well looked after fish and I wish you the best. I've seen some poor looking rescue fish but never a synodontis looking so starved.
As said by jools often the best start to recovery is simply clean water and a decent diet.

Specialist forums are a far cry from generic bragging sites like Facebook where number of 'friends' counts above all else. They may go where the food is, however nowadays there is a trend for aquariums to be almost too clean and perfect and imo this is one instance where the overzealous nature of some keepers is actually forcing the fish away from their preferred food source. My otos are frequently seen on the substrate and horizontal surfaces that grow algae or catch food. This is as well as ths expected flat , vertical surfaces including not just tank walls, wide leaf plants etc but also wood and Vallis. I also have a fair few Hisonotus leucofrenatus 'Niger/Black otos'...

If in any doubt all - avoid. It may only be a 1 in a 10000 chance of getting fish you can't house but then you have the problem of rehoming them, and if they arrive as real tank busters it can be a difficult job.
