It's only in the tiny cup during water changes and because it's way easier to photograph it when it's got nowhere to hide The hospital tank is 5 gallons with a heater. I get that isolation isn't the greatest for a shoaling species, but if I'm not sure what it is, I can't tell how contagious it is so ...
