Sick Rafael

My Rafael seems to be sick. His coloring seems blotchy. He is listless, sometimes upside down. sometimes acts like he is itchy. I had a PH problem (low) but it is OK now. He is about 6 years old. It doesn't look like Ich and is not affecting any of the other fish (pleco, spotted catfish, Silver dollar, rosy barb, knifefish) My other Rafael looks like he may be getting a little restless and listless but doesn't yet show the discoloration that this one has. See pictures. Any ideas for treating him?

sick, hungry - that is, too maeger, or both?

I would try and get it to eat first.

he is listless, sometimes lying upside down or on his side. I can nudge him and he doesn't swim away, which is so unusual. He is not hiding during the day like he usually did. He almost looks dead, but sometimes he thrashes around like he itches. He doesn't swim much either.
