Cannot add breeding report for rhinotocinclus isabelae

I was trying to put in the first breeding report for Rhinotocinclus isabelae, and upon clicking an error code shows up which I will put below. The page doesnt allow any information but the basics to be put in, and even then if you click save and publish it does not do so. I would love to add in the missing information once this is fixed.
